It's now possible to use a Google Docs spreadsheet with a simple form to set up a survey. Librarians Matter is trying this out.
In my experience, surveymonkey is an easy to use service - in fact it is very effective - but this alternative might get you around the limitations of the surveymonkey free account limits of 10 questions and no export to Excel. It's probably not going to help you out with surveys requiring logic such as: If you answered yes to Q3, go to Q6.
